Support at Home
What We Offer:
A My Aged Care assessor will determine your care needs and work with you to create a support plan tailored to your needs and goals. This funding can be used alongside other funding sources to access personalised care and support services. In certain circumstances, you may also receive interim funding for a limited period. Throughout your journey, your dedicated NAC Care Partner will provide ongoing guidance and support, conducting quarterly budget reviews to help ensure your care and services continue to align with your changing needs and goals.
If you have transitioned from the Home Care Packages (HCP) Program, you will continue to receive a funding level equivalent to your previous package. Any unspent HCP funds will be retained and can be used through Support at Home for eligible assistive technology, home modifications, and additional services you may need.
Services Provided:
If you are approved to receive ongoing Support at Home services, you will be assigned one of 8 classifications with funding. You can then access a range of services delivered by New Aged Care's staff to help you maintain your independence and continue living safely and comfortably at home. This can include support for:
Clinical Supports
Such as nursing care, occupational therapy and podiatry
Independence
Such as showering, social support, transport and respite care
Everyday Living
Such as cleaning, gardening, shopping and meal preparation
If your Home Care Package or Support at Home funding was approved after 12 September 2024, the standard contribution arrangements below will apply. If you were already receiving or had been approved for a Home Care Package on or before 12 September 2024, you are no worse off when transitioning to Support at Home.

New Aged Care also provides management for Support at Home's three short-term pathways:
Restorative Care Pathway
The Restorative Care Pathway is short-term care to help you regain function and stay independent through accessing coordinated allied health and/or nursing services. It lowers the need for more intensive ongoing services and helps you to do the activities you enjoy.
Assistive Technology and Home Modifications (AT-HM) Scheme
The AT-HM scheme gives you upfront funding to make your home safer or more accessible. It gives you access to products, equipment and home adjustments to meet your assessed needs and ensure safety.
End-of-Life Pathway
The End-of-Life Pathway will support you to remain at home and provide additional care in your last 3 months of life. You can be referred for a high-priority assessment for the End-of-Life Pathway, even if you’re not already part of the Support at Home program.
New Aged Care also provides support with Palliative Care and End-of-Life Planning. Our experienced team can help you plan for your future health, personal care and lifestyle preferences with confidence and peace of mind. As part of this service, we offer guidance and support to help individuals understand their options, discuss their wishes, ask questions and complete their Advance Care Directives in a respectful, compassionate and supportive environment.
