A General Practitioner is required to attend weekly rounds at Residential Aged Care Facilities to provide specialised services to residents with the assistance of a Nurse. Our current GPs work full time, part-time & casual. We have parents working school hours or you could even remain in your current GP role and work 1 day/week with us. Here at New Aged Care, we have the flexibility to accommodate you. We currently have work around the Macedon Ranges, Sydenham, and Glenroy areas. About the role
The Clinical Care Partner is an integral part of the Home Care team and critical to the delivery of quality care and services. Responsibilities will include but are not limited to -
- Providing clinical guidance and support, ensuring the delivery of safe, high-quality care
- Identifying and responding to clinical risks, implementing strategies to mitigate potential issues
- Delivering restorative care management as part of the upcoming Support at Home program, supporting delivery of services with wellness and reablement approaches
- Complex goal planning, working closely with the client to identify care needs, goals and preferences
- Overseeing the implementation of care and services as agreed with the client to meet their assessed needs
- Managing client home care accounts, ensuring appropriate use of funds to achieve desired outcomes
- Monitoring & responding to the changing needs of the client
- Conducting in-home assessments & reviews
- Collaborating & communicating with clients, families, internal & external stakeholders
Skills and experience
- Previous experience in case management with an understanding of the aged care industry, including Home Care Packages and the Aged Care Quality Standards
- Tertiary qualifications in nursing, OT or allied health and currently registered with AHPRA
- Strong organisational skills, with the ability to prioritise multiple tasks, meet deadlines & reporting requirements
- Sound IT skills
- Be self-motivated and able to work autonomously or as part of a multi-disciplinary team
- Strong attention to detail
- Ability to maintain confidentiality
